Ask ten people what to do for a fresh sunburn and most will mention ibuprofen somewhere on the list. What almost nobody mentions is that the drug's usefulness depends heavily on the clock. Anti-inflammatory painkillers do not treat sunburn the way antibiotics treat an infection. They interrupt a chemical cascade, and once that cascade has largely finished, the drugs shift from partially preventive to merely palliative. Understanding the timeline changes how you use them.

What is actually happening under the redness. A sunburn is not a thermal burn. Ultraviolet B radiation damages DNA and cell membranes in keratinocytes, and those injured cells respond by activating enzymes called cyclooxygenases, chiefly COX-2. These enzymes convert membrane fats into prostaglandins, especially prostaglandin E2, which dilates skin blood vessels, sensitizes nerve endings, and recruits immune cells. Nitric oxide and inflammatory cytokines such as interleukin 6 join in. This is why sunburn redness is delayed: erythema typically begins two to six hours after exposure, peaks around 12 to 24 hours, and fades over several days.

Where NSAIDs fit in. Nonsteroidal anti-inflammatory drugs, including ibuprofen, naproxen, and aspirin, block cyclooxygenase enzymes. If prostaglandin production is a major driver of early sunburn redness and pain, blocking the enzyme that makes prostaglandins should reduce both. Experimental studies dating back decades, in which volunteers received controlled ultraviolet doses and then oral or topical COX inhibitors such as indomethacin, found exactly that: measurably reduced redness and lowered skin blood flow, but only when the drug was on board early, generally within the first several hours after exposure.

The window closes fast. By roughly 24 hours, the inflammatory picture changes. Prostaglandins hand off much of the work to cytokines, immune cell infiltration, and nitric oxide pathways that NSAIDs do not meaningfully touch. Studies that dosed NSAIDs after erythema had already peaked found little to no effect on how red the skin looked or how quickly it recovered. The drugs still relieve pain at that stage, which is worthwhile on its own, but the idea that a late dose of ibuprofen will shorten a sunburn is not supported by the mechanism or the data.

Practical translation. If you realize you have overdone sun exposure, the most defensible time to take an over-the-counter NSAID, assuming it is safe for you, is as soon as possible, ideally before the redness fully develops. A standard adult dose repeated per label directions through the first day covers the prostaglandin-heavy phase. After the first 24 to 36 hours, continue only if you need pain relief, not because you expect it to speed healing.

What about acetaminophen? Acetaminophen (paracetamol) relieves pain and reduces fever through mechanisms centered in the nervous system, with weak anti-inflammatory action in peripheral skin. It will make a sunburn hurt less but should not be expected to reduce redness or swelling. For people who cannot take NSAIDs because of stomach ulcers, kidney disease, blood thinners, or certain heart conditions, it remains a reasonable comfort option.

Important caveats. First, NSAIDs do not undo photodamage. The DNA lesions that raise long-term skin cancer risk are formed at the moment of exposure, and no pill erases them. Some researchers have even asked whether suppressing the sunburn signal could tempt people into more exposure, since pain is the body's warning system. Treat the drug as symptom control, never as a license to stay out longer. Second, aspirin should not be given to children or teenagers with sunburn and any accompanying fever because of the rare but serious risk of Reye syndrome; stick to weight-based ibuprofen or acetaminophen for kids, and involve a clinician for any child with blistering burns. Third, stay hydrated. Significant sunburn shifts fluid into the skin, and NSAIDs stress the kidneys more in a dehydrated body.

What actually helps after the window. Once you are past day one, the evidence-supported playbook is unglamorous: cool compresses or brief cool showers, a bland fragrance-free moisturizer applied to damp skin, loose clothing, fluids, and leaving blisters intact because the roof of a blister is a natural sterile dressing. A short course of a low-potency topical corticosteroid can modestly ease discomfort on unbroken skin, though studies show its effect on healing speed is small. Seek medical care for extensive blistering, fever, chills, confusion, or signs of infection.

The bottom line. NSAIDs are one of the few sunburn interventions with a clear mechanism and supporting experimental evidence, but the benefit is front-loaded. Taken within the first few hours, they can blunt the prostaglandin surge that drives early redness and pain. Taken the next morning, they are simply painkillers. Neither use is wrong, but knowing the difference sets honest expectations, and it is a good reminder that the only intervention that truly changes sunburn outcomes happens before the exposure, not after.
